The « Tour de France » of intensive interdisciplinary pediatric chronic pain programs : a survey from SFETD paediatric committee

In France, intensive interdisciplinary chronic paediatric pain treatments exist but indications and contents are heterogeneous, with an unknown and random geographical distribution. In the context of the intensive interdisciplinary Pain Treatment (IIPT) workgroup within the IASP childhood SIG, the SFETD paediatric committee (IASP French Chapter) decided to conduct a survey on this subject.

Objective : make an inventory of existing facilities and procedures.

Method : A Google Forms questionnaire was developed by the SFETD paediatric committee exploring healthcare pathways, patients and procedures. It was sent to members and identified practitioners and institutions.

Results : The response rate was 68% (34 institutions). Respondents were MD (mostly from physical and rehabilitation medicine). Institutions were mainly rehabilitation centers for children and adolescents (0 to 18), few received young adults. They proposed integrated education in 91%. Only one third of respondents had specific procedures for chronic pain patients (Fig. 1). Patients had mostly primary and secondary musculoskeletal pain. Waiting time was less than 3 months in half of the institutions. They proposed inpatient or outpatient care. Absence from school was not a mandatory criterion. Length of stay was highly variable from 2 weeks to one year. Patients received somatic and psychic care, on an individual and family level, but treatments were always discussed during the stay. The primary criterion for successful hospitalization was functional improvement (Fig. 2).

Conclusion : IIPT do exist in France for children and adolescents but they are heterogenous and not sufficiently structured. We decided to set up a national work group on the subject to produce guidelines for IIPT programs.
